Marking health records as computerized?

What wording do you use when switching to computerized student health records? I need to create a stamp for the records to identify that they are now computerized. I was thinking of:
XYZ School
QRS State Student Health Records are computerized. Please keep computer printout with records"

"This student also has an electronic health record created using the SNAP Health Center program".

This indicates in addition to whatever paper forms you may have in their hardcopy file, they also have electronic record and which program needs to be accessed to see the record.
The school nurse is always assisting others. We're here to assist the school nurse.
